OUTREACH & EDUCATION |
The purpose of this program is to work towards
reducing infant mortality and its underlying
causes through a comprehensive community-based
effort by assuring that at-risk pregnant and
parenting women and their infants have access to
quality maternal and child health services.

Goals
-Conduct a community-based program that promotes
equal access to health care for all women,
children up to one year, and their families.
-No health status disparities among
racial/ethnic, gender, economic, and regional
groups.
-A safe and healthy environment for women,
children and families.
-Equal access for all women, children and their
families to appropriate and needed prenatal and
postpartum care within an integrated and
seamless system.
Implementation Activities
Prenatal Care Outreach that provides:
-Assessments and Referrals to facilitate access to care
-appropriate prenatal services
-continuous care coordination
-follow-up services to assure timely acquisition of prenatal
care, well baby care and immunizations
Case Management: A series of individual
interactions which provide support, advocacy and
assistance for high-risk women. Promotes the
development of factors that promote and support
health.
-Individualized assessment of client status, identify and
assist in the development of an individualized
care plan.
-Communicate with other providers and agencies as appropriate
to the clients care plan that addresses tobacco,
alcohol and substance abuse prevention.
-Direct the client prenatal and postpartum needs both in and
outside the agency.

Community Education related to pregnancy
outcomes:
Participation in special events including a
Celebrate Healthy Babies event to increase
community awareness of the problems associated
with infant mortality in the African American
community. Services to spearhead a local component of California's Black Infant Health Program. |
