our goal to provide

Immunizations
Immunizations are the keys to a
healthier childhood! Every child needs to have the right
shots at the right time. Because of immunizations,
there are fewer cases of diseases. You can learn more by
asking your doctor, nurse, or Health Department. They
can give you other types of information that will
protect your baby or child.
A Recommended Immunization
Schedule
2 months OPV, DTP, Hib, HepB
4 months OPV, DTP, Hib, HepB
6 months OPV, DTP, Hib, HepB
12 months DTP, Hib, OPV, MMR, VZV, HepB
18 months DTP, HepB, OPV, VZV
4-6 years OPV, DTP, VZV

common diseases prevented by shots:
DTP ........ Diphtheria, Tetanus, Pertussis
MMR ....... Measles, Mumps, Rubella
HepB ...... Hepatitis B
Hib ......... H. influenzae, type b
OPV ... Polio
VZV ... Chicken Pox
Don't Forget Your Immunization Card!
Remember to always bring your child's immunization card
with you when you visit your doctor for shots and
checkups. Keep your immunization card in a safe place!
